Process:
You can apply for a personalized or specialty license plate online, by mail, or in person.
- Online: Visit ServiceArizona.com to apply online.
- By mail: Download and complete Form #96-0143 (Special Plate Application) and mail it to the address provided on the form.
- In person: Visit an MVD office or an authorized third-party office to apply.
Rules:
- The maximum number of characters allowed on your personalized plate depends on the plate type.
- The Arizona Department of Transportation must approve all personalized plates.
- If your application is rejected, you can appeal the decision.
